I think it's supposed to be a pretty big world, or something. Also, they have that "megatexture" thing going on, so you obviously need MEGAdrivespace to install the game.

When Mr Carmack was chatting about these as quake con he sorta let on that these textures would be absolutely huge, and they could be even bugger.  There were some technical reasons behind this dealing with video drivers and frame buffers.

If only the 7000 series Radeon and GTX 600 series had 8GB on board we would see some fantastic stuff.
